  And when were the ccTLD's part of ICANN.  In fact the reality is the
other way around...  ccTLD's determination is up the the respective
countries in which those ccTLD's belong to....

ccTLD's can be inclusive or other root and registry structures should they
so choose at any time of each of their choosing...  Pretty simple to
effect really.

  True in most cases ti is true, but not all.  However with very little
restructuring and name server redirection/relocation, they could easily
do so.
