This is an excellent listing of suitable baskets.

In going through my batch of 200 responses (part of the 3000,
and not the statistical 300), I found a couple of things that
might be added:

Question 7 (How harmed)
---------------------------

The subtitle might be more precise if it reads:
"Inability to contact right party on following issues"

I would add the following to the list of issues:
- Missed domain name purchasing opportunities (expiry data problem)

Question 7 (Improvements)
----------------------------

I would add "periodically" to "validate data"

Question 8.1 (Inadequate)
--------------------------

I would add:
- Domain name history (previous owners/dates)
- For Sale availability of domain name where holder so indicates.

Question 8.1 (Unnecessary)

Comment - "Contact should only be available as some kind of
second-level enquiry, so that it is not easily available". (This is
an interesting suggestion, and is currently applied in the Chile NIC)

Question 13

I would add:
- ICANN regulation/International treaty
